Welcome Letter
WELCOME
Our goal at Healing Steps Behavioral Health is to provide you an effective therapeutic experience. Please take the time to review the policy information provided below.
PATIENT ACCOUNT
You may have initially contacted me through phone, contact forms, word of mouth or via Google email (G mail). After I receive your information (full name and phone number), I will create a user name and password. I will contact you via email or phone. Then you will need to click on the new patient link and complete the forms online. You can change the password but not the user name. Please secure your password. Your password will serve as an electronic signature. You can watch this video as a guide: https://www.youtube.com/watch?v=HbqDwrDpWug
FORMS AND APPOINTMENT SCHEDULING
Please complete all forms before your appointment. The goal of the appointment is to focus on hearing your story and understanding your journey. After you have completed the forms, click on schedule for new appointment (60 minutes, if you are a new patient) or a follow up appointment (30 minute, if you a return patient) or therapy (45 minutes, follow up with talk therapy) time slots to schedule your appointment. Please chose the right time slot. If the time slot you prefer is not available, send me a secure message and I will work with you to find a time slot that works for you. There will be Saturday and possibly Sunday appointments.
Make sure you use the invitation key hea-3775 when you sign up.
PAYMENT
We accept debit and credit cards. Your credit card will be billed when you schedule your appointment. Credit cards have higher processing fees than debit cards. This will be included in your final fee.
PRIVACY
Your information will not be discussed or shared with anyone or any institution without your permission.
MANDATED REPORTING
As a mandated reporter, I must report suspicious child, elderly abuse and maltreatment to the authorities. I will take action to keep you and others safe in the face of threats of harm to self or others.
TREATMENT TERMINATION
Although you may end treatment at any time, I firmly recommend that you share your desire to end treatment and process this decision in at least one final face to face meeting, rather than terminating by phone or mail. If for any reason, I have to terminate my relationship with you as your doctor, I will extend the same courtesy of having a discussion with you. I will make sure you have 2-3 month of non – controlled medication and one month of controlled medication. I will also provide you with a list of resources to find another psychiatrist that may be better suited for you and your level of complexity.
EMERGENCIES
If you have an urgent concern, you can log on to the patient portal and schedule an in person or tele-psych appointment. Please watch video: https://www.youtube.com/watch?v=jRRYE-0z4nQ
If none of the available time slots meet your needs then contact me via patient portal as soon as possible and I will provide you with options based on your needs.
Mental health emergency services are available through: 911, G crisis line call: 1800-715-4225 and Suicide prevention line: 1800-273-8255.
Thank you for taking the time to review these policies. I look forward to working with you.
Dr. Njoku Animashaun (aka Dr. A)
